Online ahead of print.ABSTRACTBACKGROUND & AIMS: Mother's own milk (MOM) is the preferred source of nutrients for infants, including essential minerals and trace elements. Pasteurized donor human milk (PDHM) is the preferred supplement when there is insufficient volume of MOM for infants born preterm and of very low birthweight (VLBW). VLBW infants have high nutrient requirements, so a multi-nutrient human milk fortifier (HMF) is often added to milk; however, there is a paucity of data investigating the adequacy of PDHM in meeting mineral requirements. This study aims to determine mineral and trace element concentrations in PDHM at volumes typically fed and compare intakes to expert recommendations for VLBW infants and reference values for healthy term-born infants.METHODS: 196 batches of pooled PDHM were collected as part of two clinical trials. Elements (Na, K, Ca, P, Mg, Zn, Cu, I, Mn, Fe) were quantified by ICP-MS. Enteral element intake was calculated using six commercially available HMFs and one powdered post-discharge preterm formula at 160 mL/kg/day intake and compared to the European Society for Paediatric Gastroenterology Hepatology and Nutrition (ESPGHAN) recommended intake ranges for VLBW infants. Element concentrations were also compared to reference values for term-born infants used to set the Adequate Intake (AI) values and the recent Mothers, Infants, and Lactation Quality (MILQ) study.RESULTS: The median (IQR) concentrations of sodium (94.2 (83.7-107) mg/L) and zinc (1.50 (1.22-1.84) mg/L) in PDHM did not meet ESPGHAN recommended intakes for VLBW infants (69-115 mg/kg/d and 2-3 mg/kg/d, respectively) when feeds were prepared with most HMFs assessed and fed at common fluid intakes. Intakes of copper may be low, whereas manganese and iodine may exceed recommended intakes for VLBW infants, depending on the HMF used. All elements measured except copper differed statistically from reference values for term-born infants used to create Adequate Intake values and values from the MILQ study.CONCLUSION: These results highlight the potential for suboptimal element intake when feeding fortified PDHM to preterm VLBW infants and provide valuable information for clinicians during provision of PDHM to term-born infants.
