The device includes a memory storing an ePCR including a plurality of data fields, an output device, a microphone configured to acquire speech, and a processor. The processor is configured to convert the speech to text, identify a first value of a data field of the plurality of data fields based on the text, populate the first data field with the first value, generate a prompt that requests a second value of a second data field of the plurality of data fields based on the first data field, and present the prompt to the caregiver via the output device.
